Among 74 honored out of 429 in stateSACRAMENTO—State Superintendent of Public Instruction Tony Thurmond announced today that 74 schools throughout the state were recognized as Model Continuation High Schools (MCHS) for 2025. These schools provide comprehensive services to at-risk youth through exemplary instructional strategies, flexible scheduling, and guidance and counseling services. Continuation schools provide a high school diploma program for students who have not graduated from high school, are required to attend school, and are at risk of not completing their education.
Four of those schools are located in Contra Costa County:
• Antioch Unified School District, Bidwell High School in Antioch
• Mount Diablo Unified School District, Olympic High School in Concord
• San Ramon Valley Unified School District, Del Amigo High in San Ramon
• West Contra Costa School District, Sylvester Greenwood Academy in Richmond
“Today, we celebrate 74 commendable schools for their tremendous efforts as alternative campuses of learning,” said Superintendent Thurmond. “The priority of our Model Continuation High Schools is to give students more than a diploma. The teachers and administrators aim to provide students with a student-centered approach that meets their diverse academic, social, and emotional needs and opportunities to explore options beyond high school, preparing students for the future whether they choose to pursue further education or join the workforce.”
Currently, there are 429 continuation high schools who serve close to 49,000 students throughout the state. MCHSs excel and provide exceptional opportunities for their students to pursue academic and social success.
The Model Continuation High School Recognition Program is a collaborative partnership between the California Department of Education (CDE) and the California Continuation Education Association Plus (CCEA Plus). Schools operate their own exemplary programs, but Model Continuation High School–recognized educators commit to supporting and mentoring their peers in other local educational agencies.

Six Kaiser Permanente Northern California hospitals are among the top hospitals in the nation for providing safe, high-quality patient care, according to the Leapfrog Group’s 2024 Top Hospital award.
The highest-performing hospitals on the Leapfrog Hospital Survey are recognized annually with the prestigious Leapfrog Top Hospital award, considered one of the most elite and competitive honors a hospital can receive. In 2024, only 134 hospitals nationwide, or less than 6% of the nearly 2,400 eligible hospitals, were recognized.
The six Kaiser Permanente Northern California hospitals to receive the award include: Antioch, Manteca, Modesto, San Jose, San Rafael and South San Francisco. The Leapfrog Group also recently recognized 14 Kaiser Permanente Northern California hospitals with an “A” rating for patient safety.
Kaiser Foundation Hospital Antioch was recognized as a Top Teaching Hospital.

Only 29 California hospitals received the Leapfrog Top Hospital award. Kaiser Permanente Northern California’s 6 hospitals represent more than 20% that were recognized.
The Leapfrog Top Hospital award is based on excellence in upholding quality standards across several areas of patient care including staffing, hand hygiene, infection rates, practices for safer surgery, maternity care, and error prevention. Hospitals must have also received an “A” Leapfrog Hospital Safety Grade in the most recent round of scoring to be eligible for the Top Hospital award.

The annual Top Hospital award is given to both teaching and general medical centers. Kaiser Permanente Antioch, San Jose, and South San Francisco are named top teaching hospitals and Kaiser Permanente Manteca, Modesto, and San Rafael are named top general hospitals.
The Leapfrog Group is an independent national nonprofit run by employers and other large purchasers of health care benefits. It is an independent advocacy group working with a broad range of partners, including hospitals and insurers.

SAN RAMON, CA.— Hill Physicians Medical Group, the largest Independent Physician Association (IPA) in California, has earned three prestigious honors for healthcare excellence, the organization announced on Dec. 5.
Two of the awards come from the Integrated Healthcare Association (IHA), which recognized Hill Physicians for its outstanding performance in the Sacramento and East Bay regions. In addition, Hill Physicians earned the top “Elite Status” designation in America’s Physicians Groups (APG) Standards of Excellence Survey, the highest honor the program offers. This marks the 17th year Hill Physicians has received the top ranking.
“These awards highlight Hill Physicians’ dedication and commitment to achieving the highest quality care while effectively managing costs for both patients and physicians alike,” said Hill Physicians Chief Medical Officer K. To qualify for the IHA Excellence in Healthcare awards, provider organizations must rank among the top 50 percent for clinical quality, patient experience and cost-efficiency for their commercial HMO populations. Only 22 out of nearly 200 eligible provider organizations in the state received the award, part of IHA’s “Align. Measure. Perform. (AMP)” program.
The APG’s “Elite Status” recognizes organizations excelling in value-based care models, including Medicare Advantage and the Medicare Shared Savings Program. This year, 119 IPAs applied for the award, and only 68 received the elite designation. Elite Status organizations are among the nation’s highest-performing healthcare organizations, excelling in primary care, patient-centered care and population health management, according to APG President and CEO Susan Dentzer.
APG’s Clinical Quality Leadership Forum adds new elements to each category every year in order to set a higher standard than the previous year, to continue to raise national health care standards and increase expectations of government agencies, purchasers, payers and patients.
“The capabilities that these groups possess in such areas as providing advanced primary care, delivering patient centered care, managing population health and being transparent about their quality and costs, place them at the forefront of U.S. physician practices and health systems,” said Dentzer.

Contra Costa County District Attorney Diana Becton was honored on Wednesday, Oct. 30, 2024, by the San Francisco Area Chapter of the National Organization of Black Law Enforcement Executives (NOBLE) with their Achievers Award at their 3rd Annual Achievers Luncheon.
The event was held at Scott’s Seafood Restaurant in Oakland and had the theme of “Staying the Course, Supporting Justice for the Next Generation.” During his welcoming remarks Chapter President Ja’Son Scott, Deputy Chief of the BART Police Department, said the theme, “emphasizes our collective commitment to fostering a brighter future for our leaders in law enforcement and our communities.”
“Today, we come together not only to celebrate the remarkable achievements of our honorees, but also to reaffirm our dedication to justice and equity for the next generation,” he continued. “Your presence here is a testament to our shared mission and the important role each of us plays in shaping a more just society.”
The luncheon’s Keynote Speaker was Jennifer L. Eberhardt, Ph.D., Professor of Psychology at Stanford University and author of Biased: Uncovering the Hidden Prejudice That Shapes What We See, Think, and Do.
In a Nov. 18th post on her campaign Facebook page Becton wrote, “I am deeply humbled and honored to be recognized by NOBLE, National Association [sic] of Black Law Enforcement, San Francisco Area Chapter, with the esteemed Achievers Award, alongside distinguished honorees Janieth Glenn-Davis, Jonni Redick, Sean Washington, and Margaret Dixon.”
Glenn-Davis is the retired Deputy Chief for the BART Police Department, Redick is the retired Assistant Chief of the California Highway Patrol and Washington is Chief of Police for Fremont. Dixon was honored with the NOBLE Partner Award and is a retired Oakland Police Officer.
In addition, that same day Becton shared, “Law enforcement representatives from the Bay Area participated in a discussion with me, offering expertise on human trafficking activities, trends, and enforcement tactics.”
According to the organization’s website, founded in September 1976, “The National Organization of Black Law Enforcement Executives™, also known as NOBLE®, consists of community leaders united in our mission to promote justice, equity, and excellence in the administration of justice.” Their slogan is “Justice by Action.”

This year, the Kaiser Permanente Medicare health plan in California received 4.5 out of 5 stars, according to the Centers for Medicare & Medicaid Services (CMS) 2025 Star Ratings. In addition, the Kaiser Permanente Medicare health plan in California is tied for the highest star rating in the state.
“These ratings demonstrate our commitment to providing high-quality care and exceptional service to all of our Northern California members, including our Medicare Advantage members,” said Carrie Owen Plietz, FACHE, president of Kaiser Permanente’s Northern California region. “Kaiser Permanente is routinely recognized as being among the nation’s best for improving the health and well-being of our members and patients.”
The Kaiser Permanente Medicare health plan in California has been highly rated – with mostly 4.5 and 5 star ratings – for the past 15 years.

Ratings reflect high-quality care and service
Every year, CMS publishes Medicare Advantage (Part C) and Medicare prescription drug plan (Part D) ratings. The ratings offer consumers a way to compare the quality of Medicare plans.
To develop the ratings, CMS considers many areas of care and service. For example, it looks at how well health plans keep members healthy and manage their chronic conditions. CMS also considers patient experiences, customer service, patient access, and pharmacy services.
Kaiser Permanente provides care to more than 1.9 million Medicare members in 8 states and the District of Columbia.
Northern and Southern California are rated together as one contract with CMS. Kaiser Permanente’s Dual Eligible-Special Needs Plan (D-SNP) H-contract (H8794) began in California on January 1, 2024, and is too new to be measured for the 2025 Star Ratings. It will be eligible beginning with the 2026 Star Ratings if the contract has sufficient enrollees and measures rated.

Medicare’s annual enrollment period for all 2025 Medicare health plans begins on October 15 and runs through December 7, 2024.

Kaiser Permanente had more 5-star or 4.5-star plans than any other health care organization for the ninth ratings cycle in a row.

NCQA is a national, private not-for-profit organization that surveys health plans for performance in a wide range of clinical service measures including consumer experience, prevention, equity and treatment.
NCQA primarily used the 2023 Healthcare Effectiveness Data and Information Set, or HEDIS®, to establish its ratings. HEDIS is the most widely used performance measurement tool in health care.

On Thursday, August 22, 2024, Letter Carrier Raul Hernandez will be honored during a celebration party for his 30 years of service.
Hernandez, a Pittsburg resident, began his federal service as a Marine. He later joined the Postal Service in 1998 as a Letter Carrier in San Francisco where he worked for 18 years before transferring to Martinez, CA.
A typical workday for him involves casing mail, pulling down mail, sorting packages for his route and delivering mail. “It’s been a good run here at the post office. It was kind of fortuitous that I would one day work here since I worked as a postal clerk in the Marines,” he said. “The secret to my longevity has been my military background, the word quit isn’t in my vocabulary. I truly enjoy my job and have no plans of leaving.”
The celebration will be held at 8:00 am at the Martinez Post Office, 4100 Alhambra Avenue in Martinez.

The following Kaiser Permanente Northern California hospitals have received both the AHA Get With The Guidelines®– Heart Failure and the Get With The Guidelines®– Stroke Gold Plus awards: Antioch, Richmond, Walnut Creek, Fremont, Fresno, Modesto, Oakland, Redwood City, Roseville, Sacramento, San Francisco, San Jose, San Leandro, San Rafael, Santa Clara, Santa Rosa, South Sacramento, South San Francisco, Vacaville and Vallejo. Before discharge, patients should also receive education on managing their heart failure and overall health, get a follow-up visit scheduled, as well as other care transition interventions.
The Get With The Guidelines®– Stroke award is earned by meeting specific quality achievement measures for the diagnosis and treatment of stroke patients at a set level for a designated period. These measures include evaluation of the proper use of medications and other stroke treatments aligned with the most up-to-date, evidence-based guidelines with the goal of speeding recovery and reducing death and disability for stroke patients. Before discharge, patients should also receive education on managing their health, get a follow-up visit scheduled, as well as other care transition interventions.
